Forms to surrender, request replacement or change name or address

Both Premises Licence holders, Designated Premises Supervisors (DPS) and Personal Licence holders are required under the Licensing Act 2003 to notify us as soon as possible of any changes to names and/or addresses.

Please find a number of e-forms to assist you with notifying this authority (linked to the payment facility where appropriate) of the following:

Personal Licence - Notification of a change of Name or Address (fee payable). Please note that if you are named on a Premises Licence and also hold a Personal Licence issued by this Licensing Authority, you will need to amended the Premises Licence also by completing e-forms Personal Licence - Notification of a Change of Name or Address and Licensed Premises - Notification of change of details;
Personal Licence - loss/replacement (fee payable). Personal Licence holders must notify the authority immediately if their licence/certificate is lost, stolen, damaged or destroyed and request a replacement ;
Licensed Premises - Notification of change of details (fee payable). Changing of Trade Name, change of Registered Company name (where the Company Registration number remains the same) and a change in the address of the Registered Company (as displayed by Companies House) ;
Surrendering a Premises licence, Club premises certificate or Personal licence (no fee).

If an individual wishes to cease being the DPS at a particular premises, please notify this Authority using form L45.
